Ning, female, 60 years old, treated on December 15, 1968. She has a history of asthma and cough disease for more than 20 years, with symptoms worsening in winter and alleviating in summer, and attacks triggered by cold. She was diagnosed with bronchiectasis, lung qi swelling, and pulmonary tuberculosis, and was treated with anti-tuberculosis and anti-infection medications, with symptoms fluctuating and lingering without cure. In the past two years, she developed complications such as palpitation, panting, and edema. In severe cases, she experienced cold limbs, cyanosis, dysuria, and a pulse rate of over 120 beats per minute. Western medicine diagnosed her with cor pulmonale, but treatment with cardiotonic, diuretic, and anti-infection drugs was ineffective. Treatment based on the theory of insufficiency of heart yin, using Pulse-Reinforcing Powder with yin-enriching ingredients, also failed and even worsened her condition. She then sought advice from the master. The master said: "This is not insufficiency of heart yin, but rather the failure of middle yang to circulate, leading to the stagnation of water-dampness. Now, using yin-enriching methods, water qi attacks the heart, and cold water shoots into the lungs, exacerbating the panting, cough, and edema. The treatment should focus on dispersing the upper and middle, guiding water downward, and eliminating it through both the front and back, while also warming yang." He then prescribed Stephania, Pricklyash Seed, Pepperweed, and Rhubarb Pill with Aconite Lateral Root. After taking the medication, her cough and panting lessened, the edema subsided, and other symptoms improved.
In winter, as her cough and panting recurred, accompanied by chest tightness, shortness of breath, and worsened panting, she followed the master's previous prescription and continued with Stephania, Pricklyash Seed, Pepperweed, and Rhubarb Pill. After taking the medication, her complexion turned pale, she developed generalized edema, relied on breathing support due to severe panting and cough, experienced chest tightness, palpitation, cold limbs, cold sweating, dysphoria, clear and copious urine, loose stools, cyanosis, and coughed up blood-streaked sputum. Her tongue was pale with a white coating, and her pulse was deep, thin, and rapid at 124 beats per minute. The master said: "The syndrome is different, so the mechanism of disease is also different. This syndrome is due to insufficient true yang. How can one indiscriminately use purging methods? The treatment should focus on restoring yang to save from collapse, and it is necessary to use herbs that reverse cold limbs to save a life in critical condition." He then prescribed:

Poria, processed Aconite Lateral Root, Dried Ginger, each 30g, Prepared Liquorice Root, Cinnamon Twig, each 15g, Korean Ginseng 12g
He instructed her to prepare a strong decoction and take it frequently. After taking one dose, her sweating stopped, yang returned, her limbs warmed, her cough and panting lessened, her dysphoria ceased, and her pulse rate dropped to 96 beats per minute. She continued taking the prescription for 15 doses, and her symptoms improved. After further treatment, she recovered and was able to participate in light labor.
