Balanoposthitis refers to the inflammation of the inner foreskin and the glans penis. Normally, the foreskin cavity secretes a lipid-like substance. In cases of redundant prepuce or phimosis, this substance can accumulate to form smegma, which irritates the foreskin and glans, leading to balanoposthitis. Drug-induced balanoposthitis is a delayed-type hypersensitivity reaction, which is quite common clinically. It typically occurs 24 to 72 hours after medication use.

bubble_chart Clinical Manifestations

Balanoposthitis manifests as redness and swelling of the foreskin, burning pain that worsens during urination, and possible purulent discharge from the foreskin opening. If the foreskin is retracted, the inner foreskin and glans appear congested and swollen; in severe cases, there may be superficial ulcers or erosions with pus present.

Drug-induced balanoposthitis caused by drug allergies is a delayed-type hypersensitivity reaction, which is quite common clinically and typically occurs 24 to 72 hours after medication use.

bubble_chart Treatment Measures

For balanoposthitis, the affected area can be soaked in a 1/5000 potassium permanganate solution and treated with anti-inflammatory ointment. Allergic balanoposthitis requires oral antihistamines and topical corticosteroid ointment. If phimosis or prepuce skin edema prevents retraction and thorough soaking, or if drainage is inadequate and inflammation persists despite general treatment, a dorsal slit of the prepuce (Figure 1) may be performed to facilitate drainage. Circumcision should be performed only after the inflammation has completely subsided.

Regularly clean the foreskin and glans to maintain cleanliness and dryness inside the foreskin cavity. If there is excessive foreskin or phimosis, circumcision should be performed.
