Anger arises in the heart and affects the liver, hence when angry, liver qi responds. Anger harms the liver, leading to liver qi depression and disorder as the main pathological changes. "The liver is the organ of the general, from which strategy and planning emerge." Anger causes qi counterflow, and as qi counterflows upward, blood follows, filling the facial vessels, causing facial redness and fever, qi turbulence in the chest, clenched fists, rapid breathing, general body heat, and fearlessness, hence the liver forgets anger. Anger can inhibit normal thinking, often leading to impatience, agitation, mental confusion, and loss of strategic thinking. Or, due to liver qi depression and rebellion, it manifests as pain and distension in the flanks, aversion to heat, headache, chest and epigastric fullness, poor appetite, nausea, and hiccups; then liver qi counterflows upward, causing belching, vomiting, qi fullness, and even liver syncope with unconsciousness, locked jaw, cold extremities, and loss of consciousness. This is the "emotional syncope" caused by great anger as described in Suwen-Shengqi Tongtian Lun. Qi counterflow to the bladder results in bladder colic and pregnant dysuria, sudden urinary retention, and rising qi impacting the heart. Qi counterflow transforming into fire causes vertigo, bitter taste, palpitations, spasms, acid regurgitation, flank pain, sore throat, or swelling around the ears. Qi disorder leads to gall bladder leakage, causing bitter taste, fever, and yellow urine. Liver fire punishing metal causes sore throat, hoarseness, and cough with hypochondriac pain. If qi depression produces phlegm, phlegm-heat disturbs the heart, leading to insomnia and irritability; attacking the stomach causes hiccups and poor appetite; attacking the lungs causes panting and qi counterflow, coughing with phlegm; phlegm qi depression and upward counterflow cause throat obstruction, leading to globus hystericus; phlegm clouding the pericardium causes mania; phlegm-fire disturbing the heart causes madness. If liver depression and qi stagnation lead to qi stagnation and blood stasis, the pulse becomes rough and deep, causing blood stasis with stabbing pain, and abdominal pain with distension. Stasis below the heart causes liver heart pain, with a pale complexion and inability to sigh. Stasis above the heart causes lung heart pain, pain starting from the heart, worsening with anger. Static blood following rising qi affects the heart, leading to loss of mental activity, causing angry madness and heat reversal. Or, stasis in the skin causes Yongju, or stasis in the abdomen leads to abdominal masses and accumulations. Additionally, prolonged liver qi depression transforming into fire internally damages yin blood, and since the liver and kidney share the same source, liver and kidney yin blood deficiency may occur, or even liver and kidney yin and yang deficiency, which is difficult to recover from.
