When the coating on the tongue appears white, it is called "white tongue coating." The character "苔" (coating) was also written as "胎" in ancient times.

The "Guide to Tongue Differentiation: Diagnostic Methods for White Tongue Coatings" states: "...the tongue body is pale red, the tongue coating is slightly white, ... neither too dry nor too moist, this is the coating of a healthy person." That is, a normal person's tongue body is pale red, and the tongue coating is slightly white, which is different from pathological white tongue coating and should be distinguished. Cold is a yin pathogen, easily damaging yang qi, hence the "Guide to Tongue Differentiation" states: "A tongue without coating and moist, or with a thin white coating, indicates wind-cold, with external symptoms of aversion to cold and fever." Key points of differentiation: mild fever with severe aversion to cold, no sweating, no thirst, moist white tongue coating, normal tongue body, headache and body pain, and a floating pulse. Treatment should focus on releasing the exterior with pungent-warm herbs, using Ephedra Decoction.

White tongue coating due to cold-dampness attacking the exterior: caused by exposure to cold in the early morning, or sweating during long journeys and being drenched in cold rain, leading to cold-dampness attacking the exterior and Xieke the skin. Since the pathogen has not entered the interior, the tongue coating is white and slippery. The "Guide to Tongue Differentiation" states: "Cold-dampness is a yin pathogen, white indicates cold, hence a white and slippery tongue coating indicates wind-cold and dampness." Cold-dampness is heavy and stagnant, causing headache and heaviness in the head, heavy pain in the lower back and spine, or pain all over the body, and a tight pulse. Yang qi is obstructed and cannot be released, leading to aversion to cold and fever, and no sweating on the skin. This syndrome is similar to the white tongue coating in wind-cold common cold, but in wind-cold invading the exterior, the tongue coating is moist and white, accompanied by wind-cold common cold symptoms, while in cold-dampness attacking the exterior, the tongue coating is white and slippery, accompanied by dampness stagnating in the skin. Treatment should focus on dispersing wind and dampness, using Notopterygium Overcoming Dampness Decoction.
  3. Spleen Yang Deficiency with Pale Tongue Coating: This syndrome arises from prolonged illness leading to Spleen Yang depletion, or repeated vomiting and diarrhea severely damaging the Middle Qi, or from consuming cold leading to Cold in the Middle, causing Spleen Yang to gradually decline. Internal Cold congeals and blocks the Middle Energizer, impairing the transformation and transportation of water-dampness and the distribution of body fluids, resulting in a pale, moist, and shiny tongue coating resembling snowflakes. "The Guide to Tongue Differentiation" states: "When the tongue develops a white coating like snowflakes, commonly known as snowflake coating, it indicates Spleen Cold and obstruction." Key diagnostic points include, in addition to the white coating, cold pain in the abdomen, abdominal distension and fullness that prefers warmth, cold limbs, and a timid and cold demeanor, all signs of impaired Spleen function and internal deficiency-cold. Treatment should focus on warming the Middle and strengthening the Spleen, using sweet and warm herbs to support Yang. The formula used is Aconite Middle-Regulating Decoction with modifications.
A white tongue coating generally indicates a Cold pattern. "The Essential Meaning of Cold Damage Disease" states: "All tongue coatings that are white indicate Cold; the whiter the coating, the more severe the Cold." Cold can be divided into Exterior Cold and Interior Cold. Exterior Cold often presents with a thin white coating; Interior Cold presents with a slightly thicker white coating. If dampness is involved, the coating may appear greasy; if caused by Spleen Yang Deficiency, the coating is white, clean, moist, and shiny, resembling snowflakes. Based on clinical manifestations, differentiation is relatively straightforward. Generally, a thin white coating indicates a mild condition, a thick white coating indicates severe Interior Cold, and a snowflake coating indicates a critical state of Spleen Qi collapse. As stated in "The Tongue Mirror of Cold Damage Disease": "White coating can also indicate fatal conditions and should not be overlooked."
