The white and greasy coating is not mentioned in the "Treatise on Cold Damage Diseases." In the chapter on Yangming disease, there is a passage: "Yangming disease, with hard fullness below the ribs, no bowel movement, vomiting, and a white coating on the tongue, can be treated with Minor Bupleurum Decoction," which likely refers to a white and slippery coating.

Warm disease scholars have begun to pay attention to the study of white and greasy coatings. Xingse Waizhen Jianmo states, "Latent pathogens and seasonal pathogens both arise from within, often accompanied by dampness. Therefore, at the onset, there is already a white coating on the tongue, which is thick rather than thin, greasy rather than slippery, or coarse like accumulated powder." This indicates that white and greasy coatings are also commonly seen in latent pathogens.

Although both white and greasy coatings and white and curdy coatings are thick, they are different. The curdy coating has coarse particles and is easily scraped off, while the greasy coating has fine particles that adhere to the tongue surface and are not easily wiped off, which helps to distinguish them.

Normal people may develop a white and greasy tongue coating after consuming milk or soy milk, which is considered a stained or false coating and not a pathological condition.

The tongue coating is white, thick, greasy, and slippery, resembling thick soy milk, with a bluish-purple tongue body. During diagnosis, symptoms such as abdominal distension and fullness, poor appetite, cold extremities, vomiting of clear fluid and phlegm, and a deep, slow pulse may be observed, indicating the internal retention of dampness. "Tongue Mirror Differentiation" states: "A white, thick, powdery, moist, and greasy coating that can be slightly scraped off but accumulates like flour mixed with water indicates interior cold and damp stagnation. Using Caoguo to invigorate spleen yang can promote the rising of qi, thereby eliminating the white coating caused by dampness." The treatment should focus on warming yang, invigorating the spleen, and promoting water metabolism. The recommended formula is Spleen-Warming Decoction.
The symptom of a white greasy tongue coating has been widely recognized by medical practitioners throughout history as indicating dampness, phlegm, or cold. "Guide to Tongue Differentiation" mentions: "A white, slippery, and greasy coating indicates damp turbidity and phlegm. A thick, slippery, and greasy coating suggests damp phlegm and cold." However, the specific symptoms vary depending on the location of the pathogenic factors. In cases of external contraction of cold-dampness, the pathogenic factor is in the exterior, and the tongue coating is thin, white, slippery, and greasy, accompanied by symptoms of cold-dampness binding the exterior. In cases of dampness obstructing the membrane source, the pathogenic factor resides in the half-exterior and half-interior, with a white, thick, greasy, and dry tongue coating, or resembling accumulated powder, and a slightly red tongue body, accompanied by symptoms of dampness obstructing yang qi. In cases of cold-dampness retention, the pathogenic factor is retained in the zang-fu organs, with a white, thick, greasy, and slippery tongue coating, a bluish-purple tongue body, and symptoms of fluid retention. These three patterns can be easily distinguished based on the characteristics of the tongue coating and accompanying symptoms.
