Limb pain refers to the symptom of pain in the tendons, muscles, and joints of the upper or lower limbs, or both, experienced by the patient.
Limb pain was first mentioned in Neijing, such as "limb joint pain," "bone pain," "arm pain," "foot pain," "hip pain," and "pain in the thigh, knee, calf, and foot." The "Cold-Damage Disease Treatise" and Jingui Yaolue also record "multiple arthralgia pain," "limb pain," and "joint pain." Later generations referred to conditions such as "pain wind," "wind-induced waist and leg pain," "wind wandering pain," "shoulder and arm pain," "finger pain," "thigh pain," "foot pain," "heel pain," "foot heart pain," "leg pain," and the "Liu Guai Zi disease" found in mountainous regions as falling within the scope of limb pain conditions.
Neijing discusses Bi disease, such as "migratory impediment (migratory impediment)," "agonizing arthralgia (agonizing arthralgia)," "fixed arthralgia (fixed arthralgia)," "heat arthralgia," "sinew impediment," "vessel impediment," "muscle impediment," "skin impediment," "bone impediment," Zhoubi, and "multiple impediments," which also primarily present with limb pain as the main symptom and sign.

- Wind-evil obstructing the collaterals causing limb pain: Wandering pain in the limb joints, with no fixed location, commonly seen in the shoulders, elbows, knees, and ankles, with limited joint movement, possibly accompanied by cold or heat exterior pattern, thin white or greasy tongue coating, and a floating pulse. This belongs to migratory impediment.
- Cold-evil obstructing the collaterals causing limb pain: Cold pain in the limb joints, with a fixed location, cold limbs, local skin not red, worsening with cold and alleviating with warmth, white tongue coating, and a wiry and tight pulse. This belongs to agonizing arthralgia.
- Dampness-evil obstructing the collaterals causing limb pain: Aching pain in the joints, heavy and fixed, or numbness of the skin, leading to muscle stiffness and joint deformation if untreated for a long time, resulting in disability, white and greasy tongue coating, and a soggy and slow pulse. This belongs to fixed arthralgia.
- Pathogenic heat obstructing the collaterals causing limb pain: Pain in the limb joints, with local redness and swelling, accompanied by fever, thirst, dysphoria, red tongue with yellow and dry coating, and a rapid pulse.
- Dampness-heat obstructing the collaterals causing limb pain: Red and swollen joints, dark and turbid urine, heavy and painful limbs, slippery or soggy and rapid pulse, red tongue with yellow and greasy coating, possibly accompanied by red nodules on the skin.
- Qi and blood deficiency causing limb pain: Aching pain in the joints, worsening after exertion, possibly with muscle wasting, pale complexion, pale lips and nails, shortness of breath, fatigue, vertigo, fear of wind and spontaneous sweating, thin and weak pulse, pale tongue with thin coating. If accompanied by static blood, the joint pain is like a stabbing pain, fixed and tender, with rough skin, emaciation or joint deformation, dark tongue with ecchymosis, thin and greasy coating, and thin and choppy pulse. If accompanied by phlegm, there is pain in the arms, heavy body, head feeling wrapped, swollen tongue, white and greasy coating, and wiry and slippery pulse.
- Liver and Kidney Deficiency with Pain in the Limbs: Relaxation or spasm of tendons and bones, soreness and pain, dizziness, brittle nails, soreness and weakness in the lower back and knees, tinnitus and hearing loss, loose teeth and hair loss, impotence and nocturnal emissions, deep and weak pulse in the chi position. If there is a predominance of yin deficiency, there will be hot and painful joints that prefer coolness, bone pain that worsens at night, flushed cheeks and red lips, a bright red tongue with little coating, or a deep red tongue with little moisture. If there is a predominance of yang deficiency, there will be swelling and weakness in both feet, sloppy diarrhea, clear and long urination, and cold hands and feet.
bubble_chart Differential Analysis - The most common cause of limb pain is the obstruction of meridians by wind-cold dampness, which is known as Bi disease. Bi means obstruction, and pain arises when there is obstruction and stagnation. Suwen.Bi Lun states: "When wind, cold, and dampness combine, they form Bi disease. If wind qi predominates, it causes migratory impediment; if cold qi predominates, it causes agonizing arthralgia; and if dampness predominates, it causes fixed arthralgia." The upper and lower limbs are traversed by the six meridians of the hands and feet, making them most susceptible to invasion by wind, cold, and dampness. Therefore, Jisheng Fang.諸痺門 states: "It is due to the deficiency of the body and the emptiness of the striae and interstices, allowing wind-cold dampness to invade and cause Bi disease." However, since the predominance of pathogenic factors varies, it is essential to differentiate them clinically to establish appropriate treatment principles. Differentiation is based on the disease cause, mechanism of disease, and the nature and characteristics of the pain.
- For wind obstruction in the meridians, Zhubing Yuanhou Lun.四肢痛無常處候 states: "Wind pathogens move with qi. When qi is deficient, pathogenic qi prevails and clashes with healthy qi, causing pain that arises with deficiency and thus has no fixed location." Shengji Zonglu.諸痺門 states: "Wind is yang qi, characterized by rapid movement and change. Therefore, when wind qi predominates, it causes migratory impediment, with symptoms that move freely without fixed location, leading to obstruction of qi and blood." Pain often affects multiple joints and migrates unpredictably.
- For cold obstruction in the meridians, Shengji Zonglu.諸痺門 states: "Cold entering the meridians causes stagnation and obstruction, leading to pain. The pain is rooted in the predominance of cold qi, which results in less yang qi and more yin qi." The pain is severe and localized, often accompanied by cold limbs.
- For dampness obstruction in the meridians, Shengji Zonglu.諸痺門 states: "Dampness from the earth harms the skin, flesh, sinews, and vessels. Dampness is associated with earth, which has a slow nature. When dampness prevails, it lingers and does not move, leading to symptoms such as excessive sweating and dampness due to the predominance of yin qi." The pain is heavy, fixed, and may be accompanied by numbness and soreness.
Clinically, wind predominance often presents with exterior pattern, such as aversion to wind, fever, thin white tongue coating, and floating-tight pulse; cold predominance often presents with cold pattern, such as cold intolerance, cold limbs, clear and abundant urine, thin white tongue coating, and deep-tight pulse; dampness predominance often presents with dampness pattern, such as heavy limbs, chest tightness, white greasy tongue coating, and soggy-slow pulse. The disease cause and mechanism of disease differ among the three, leading to different treatment principles. For wind predominance, the treatment should focus on dispelling wind, assisted by dispersing cold and draining dampness, combined with blood-tonifying herbs, as treating wind requires treating blood first—when blood circulates, wind dissipates. A modified Saposhnikovia Root Decoction can be used. For cold predominance, the treatment should focus on dispersing cold, with the addition of pungent warming and tonifying herbs to release cold stagnation. A modified Aconite Main Root Decoction, dispersing wind and activating blood decoction, or Xiao Harmonizing Collaterals Pill can be used. For dampness predominance, the treatment should focus on draining dampness, assisted by dispelling wind and dispersing cold. A modified Coix Seed Decoction or Dampness-Removing Impediment-Alleviating Decoction can be used. For a combination of wind, cold, and dampness, a modified Three Impediments Decoction can be used. - Pathogenic heat and dampness-heat obstructing the meridians can also cause limb pain. The former is due to a constitution with excessive heat and yang qi, leading to internal heat accumulation. Although wind-cold dampness is also contracted, the clinical manifestations differ from wind-cold-dampness impediment. Jingui Yi states: "When there is pre-existing heat accumulation in the organs and meridians, and wind-cold dampness invades, the heat is constrained by cold, leading to qi obstruction. Over time, the cold transforms into heat, causing sudden and oppressive Bi disease." Symptoms include red, swollen, and painful joints. The treatment should focus on clearing heat, assisted by dispelling wind and removing dampness. A modified White Tiger Decoction Plus Cinnamon Twig can be used. If pathogenic heat transforms into fire, causing joint redness, swelling, and stiffness, along with high fever and thirst, a modified Rhinoceros Horn Decoction can be used. The latter is caused by external contraction of dampness-heat pathogens, pre-existing dampness combined with pathogenic heat, or dampness transforming into heat over time, leading to dampness-heat accumulation and meridian obstruction. The treatment should focus on clearing heat and drying dampness. A modified Two Wonderful Herbs Powder or Angelica Pain-Relieving Decoction can be used. The mechanism of disease for pathogenic heat or dampness-heat obstruction is excessive yang qi and insufficient yin qi, leading to prominent heat signs clinically. These can be easily distinguished from the symptoms of wind-cold-dampness impediment.
- Deficiency of qi and blood fails to warm and nourish Jingmai, resulting in pain in the limbs and joints, often accompanied by a pale complexion, emaciated muscles, fatigue, and reluctance to speak. Since blood follows qi, qi deficiency leads to poor blood circulation, Jingmai obstruction, and the syndrome must present with limb pain like a锥刺, fixed pain location, emaciated body, scaly skin, stiff joints, and瘀点 on the tongue edge. The treatment should focus on tonifying qi and nourishing the营, invigorating blood and resolving stasis. The prescription includes Taohong Yin with Astragalus Root, Tangshen, Cinnamon Twig, Sichuan Lovage Rhizome, and酌情加入虫类搜剔之品 such as全蝎, Earthworm, dung beetle,山甲, centipede, and乌稍蛇, or配合Major Collaterals-Activating Pill,小harmonizing collaterals丹, musk丸. Additionally, weak qi leads to dysfunction of the spleen in transportation, not only causing a lack of generation and transformation but also容易导致痰浊内停. The key points of pattern identification must include limb and arm pain, heavy body, and white greasy tongue coating. The treatment should focus on tonifying qi and regulating the营, clearing up phlegm and通络. The prescription includes Guishao Liujun Wan配合指迷Poria Pill.
- The liver governs the tendons, and the kidneys govern the bones. Deficiency of the liver and kidneys leads to malnutrition of the tendons and bones, clinically often manifested as flaccidity or stiffness and soreness of the tendons and bones, soreness and weakness of the waist and knees. Treatment should focus on tonifying the liver and kidneys, diffusing impediment and harmonizing collaterals. The prescription used is Pubescent Angelica and Taxillus Decoction with modifications.
The symptoms of limb pain are externally caused by wind-cold dampness-heat, and internally related to qi and blood deficiency, liver-kidney depletion, and obstruction by stasis and phlegm-turbidity. It is necessary to carefully differentiate from the aspects of disease cause, nature of pain, and accompanying symptoms, pattern differentiation and treatment, in order to achieve good therapeutic effects.
