bubble_chart Overview

Stirred pulse after cerebral sclerosis causes neuropsychiatric disorders due to multiple cerebral infarctions, softening, necrosis, and atrophy.

bubble_chart Diagnosis

1. Medical history and symptoms:

The onset is gradual, with possible histories of coronary heart disease, hypertension, hyperlipidemia, and diabetes. Symptoms include:

1. Headache, dizziness, difficulty concentrating, decreased work efficiency, and significant memory decline, often accompanied by vertigo episodes. In the early stages, the main symptoms are recent memory loss and slowed thinking. In the advanced stage, memory deteriorates, thinking becomes difficult, speech slows, emotions become dull, and forced crying or laughing may occur, eventually leading to dementia.

2. Personality changes: Various delusions such as suspicion, hypersensitivity, or hypochondria may appear. In the advanced stage, behavior may become childish or exhibit various antisocial or immoral actions.

2. Physical examination findings:

There are various degrees of neurological signs, such as hemiplegia, pathological reflexes, palmomental reflex, hemisensory impairment, asymmetric tendon reflexes, and head-hand tremors. Sometimes, pseudobulbar palsy or Parkinson's syndrome may be present. Fundus examination may reveal narrowed retinal arteries, enhanced light reflexes, and arteriovenous crossing signs.

3. Auxiliary examinations:

Cerebral blood flow imaging shows prolonged rise time, weakened or absent dicrotic waves, blunted main peak angle, and decreased amplitude. Cranial CT often reveals varying degrees of brain atrophy and infarct lesions of different sizes and numbers.

4. Differential diagnosis:

Differentiation is required from various dementias and psychiatric disorders.

bubble_chart Treatment Measures

1. Cyclandelate (400mg 3 times/day), Duxil (40mg 1-2 times/day), Cinnarizine (25mg 3 times/day), and Papaverine (60mg 2 times/day) can improve cerebral blood circulation. Additionally, invigorating blood and resolving stasis Chinese medicinals and 706 plasma substitute (500ml intravenous drip, once daily) can be used.

2. γ-Aminobutyric acid, ATP, Coenzyme Q10, Sermion, and Citicoline can improve brain metabolism.

3. Lipid-lowering drugs such as Zhibitai, Fenofibrate, and Zocor can be used.

4. For anxiety, Diazepam or Clonazepam can be administered. For excitement, delusions, or hallucinations, Perphenazine or Haloperidol may be used. For depression, Deanxit, Prozac, Seroxat, or Imipramine (used with caution in patients with poor cardiac function) can be prescribed. For Parkinson's syndrome, Levodopa and similar medications may be employed.
